What we'd like to see in an application:

Demoreel with, preferably a shot breakdown (thumbnails or timecode in one column: description what you did, in the other)
Portfolio, online link or files
CV: education and on what projects you worked on until now.
Cover letter, telling us who you are, with your own words. And why you want to work for us.

Example for a shot breakdown-description:
"I did shading for the armor of the hero-character, precomp for the background, and the lighting effects on the weapon. (again, shot thumbnail and/or timecode on the left...)"

Tips for the Application:

Put in only your top quality work
Put best work first on the reel.
Short and good, better than long and tedious.
Keep it short and concise, we can see more later when we meet with you for the first time.
Only display your capabilities and clearly indicate your role on group projects.
